Appointments for our satellite office in Delaware are scheduled by calling that office directly.

Some new patient paperwork may be mailed or faxed to you for completion prior to your visit. Completion of these forms will greatly expedite your office visit. If you are unsure of how to answer any questions, simply skip them, and the office staff will gladly assist you at the time of your office visit.

Please arrive at least 15 minutes earlier than your scheduled appointment time to assure accurate completion of all new patient paperwork.

Please bring all of your medication vials with you at the time of your office visit. A list is not an acceptable substitute. We have discovered many discrepancies between lists and actual medication regimens in the past. It is our goal to provide you with optimal medical care and we feel strongly that an accurate accounting of your medication regimen is crucial in this process.

The doctor will review your referral records and may request additional lab tests, x-rays, or medical records prior to your office visit. This is done to make your visit more productive. If so, you will be notified and sent the appropriate forms. Lab tests can be done at any local laboratory your medical insurance allows.

Unless you request otherwise, the doctor will send a letter to your referring physician outlining the diagnosis and treatment of your condition. This letter should arrive within 1-2 weeks of your visit.
