Permit testing is no longer availale at the driving school

Please contact the DMV for permits As a convenience we list below the documentation that is currently needed.

• Copy of Birth Certificate
• Copies of two other valid forms of I.D. including Social Security card, Passport Verification, present address (envelope with current postmark date) DMV application.
• A clear photo of the person obtaining the permit (this photo will not be returned)
• Notarized parental consent form. This form is necessary for 16 and 17-year-old students ,
